All survey responses are strictly confidential.  Data collected is used solely for the purposes of applying for corporate-sponsored or federally-funded grants and in keeping in compliance with the requirements of those grants.

Please do not take this survey unless you have participated in an Arts For All class in either 2021 or 2022.
